Set up the Occupational and/or Speech Therapy Evaluation

STEP 1

Click the Contact Us link and send us a message. You will soon receive a Medical Intake form from us.
After reviewing your child's medical intake information, the Registered Occupational Therapist (OTR) and/or Speech Language Pathologist (SLP) will contact you to set up the initial evaluation.
Your child will participate in the initial evaluation with a friendly OTR and/or SLP, undergo appropriate standardized testing based on your child's level of participation, and be introduced to our engaging therapy clinic.
Your child will also get to meet and interact with the Certified Occupational Therapy Assistant (COTA) during the initial evaluation if they are receiving OT services.
With your collaboration, the Therapy Team will establish a plan of care with specific attainable goals for your child.

Start Occupational and/or Speech Therapy Treatments

STEP 2

The COTA or Speech Therapist will set up ongoing therapy treatment sessions according available schedules.
The treatment sessions will be customized with your child's needs in mind at all times ranging between 30-45 minutes.
The Therapy Team will continually communicate about the strengths, weaknesses, goals, and treatment plans for your child.

STEP 3

The Therapy Team will collectively complete a chart and data review, consult with your regarding the progress of your child, re-evaulate the plan of care, establish new goals if necessary, or plan for a successful discharge.
After your request and final report payment is collected, the OTR or SLP will submit a discharge summary for the referring pediatrician or for your child's home records.

~We serve all children ages 3 through 18 years of age.

~Cash pay, debit card, or credit card only.

~All payments due at time of service; no payment plans.

~If a treatment appointment is not cancelled by the parent/guardian within 24 hours, a $50 fee will be charged via invoice; if an evaluation appointment is not cancelled by the parent/guardian within 24 hours, a $110 fee will be charged via invoice.

~All evaluations, treatment sessions, and meetings are by appointment only.

~ Closed for all federally recognized holidays.

NOTE: We do not accept insurances, SoonerCare or Medicaid. You will receive a copy of the evaluation, treatment sessions, and sales receipt for you to submit to your insurance company for a claim/reimbursement.

Written Reports (after initial evaluation is complete)	Flat rate per report	$100.00	Any written report (besides the initital evaluation and goals) for your child's peditrician, insurance company, personal files, school, etc.
Telephone calls and emails	Billed in 15-minute increments	$25.00	Phone calls requested by parents/guardians to peditirician, social worker, nurses, teachers, special ed directors, and any other out-of-clinic contacts.
